CCR and affiliated centers offer a supportive environment to all Residents/Patients, Family members and Associates who report concerns or problems.

Associates may directly contact their immediate supervisor, department manager, Senior HR Generalist or Administrator.

Residents/Patients and family members may directly contact the Administrator and/or Director of Nursing.

However, if your concern is unresolved or you wish to remain anonymous, please contact the ACHIEVE HelpLine through LIGHTHOUSE, an independent, 3rd party service, that will assist you to anonymously report your concern.

All reported concerns are confidential and will be monitored through to resolution. CCR policies prohibit all retaliation or retribution against anyone who reports a concern.
